Where you'll be working
Tomaree Community Hospital is a valued part of the Hunter New England Local Health District, nestled in the picturesque Port Stephens region of New South Wales. We offer a warm, community-focused healthcare environment built on the principles of patient-centred care, safety, and wellbeing.
Our hospital strikes a unique balance — small enough to foster strong connections and a sense of belonging, yet large enough to provide diverse clinical experiences and professional development opportunities. At Tomaree, our staff thrive in a supportive, multidisciplinary team culture, where collaboration and compassion drive every interaction. Together, we make a meaningful difference in the lives of our patients, their families, and the broader community.
What you'll be doing
We are seeking a dedicated and compassionate Social Worker to join our team on a permanent part-time basis at Tomaree Community Hospital. In this rewarding role, you will deliver professional social work services across both inpatient and Emergency Department (ED) settings, providing essential support to patients and their families during times of need.
Your work will involve comprehensive psychosocial assessments, complex discharge planning, counselling, advocacy, and referrals to appropriate community and support services. You will collaborate closely with our multidisciplinary team to ensure that care is holistic, coordinated, and responsive to each individual's unique circumstances.
As a valued member of our team, you will play a pivotal role in enhancing patient outcomes, promoting emotional wellbeing, and ensuring that every person in our care experiences dignity, respect, and empowerment. This is an excellent opportunity for a motivated professional who thrives in a community-focused, inclusive healthcare environment, and who is passionate about making a meaningful difference in the lives of others.
About you
We are looking for an experienced and motivated Social Worker who is committed to delivering high-quality, patient-centred care. You will bring strong communication and teamwork skills, enabling you to engage respectfully and effectively with patients, families, and colleagues across all areas of the hospital. Your proactive approach to problem-solving and advocacy will support complex discharge planning and help ensure positive, well-coordinated patient outcomes. A commitment to ongoing professional development, ethical practice, and the values of NSW Health will be essential to your success in this role.
