About the RoleAs an NDIS Support Coordinator, you will work closely with participants, their families, and service providers to coordinate supports, overcome barriers, and achieve the goals outlined in their NDIS plans. This role requires a proactive, compassionate, and highly organised professional who thrives in a dynamic environment.
Key Responsibilities- Assist participants in coordinating their supports and achieving their NDIS plan outcomes
- Provide timely and high-quality support coordination services
- Develop complex service plans to address barriers and implement well-coordinated solutions
- Liaise with NDIA, NDIS Quality and Safeguards Commission, and other key stakeholders
- Monitor budgets, prepare reports, and complete all administrative tasks
- Conduct risk assessments, emergency plans, and support plans to a high standard
- Facilitate access to services, including transitions from custody to community
- Advocate for participants' rights and ensure culturally sensitive, inclusive support
- Maintain accurate documentation using systems such as ShiftCare and PRODA
- Identify service improvements and evaluate marketing opportunities
- Conduct plan reviews and ensure compliance with all mandatory reporting requirements
- Minimum Diploma in Community Services with case management experience
- Tertiary qualifications in Social Work, Occupational Therapy, Allied Health, or equivalent (preferred)
- Registration with relevant professional body (if applicable)
- Strong understanding of the NDIS and experience working with people with disability
- Excellent communication, negotiation, and stakeholder engagement skills
- High attention to detail and ability to manage competing priorities
- Current Criminal Record Check
- Current Working with Children Check
- Completion of the NDIS Worker Orientation Module
- 100 Points of ID
- Valid Driver's Licence (if travel is required)
- Experience supporting individuals with complex needs, including trauma, substance use, or justice involvement
- Familiarity with assistive technology and accessibility tools
- Ability to support participants in employment, education, and community engagement
