Your Qualifications

We are looking for a qualified Aboriginal and Torres Strait Islander Health Professional that:

  • Aboriginal Health Practitioner: Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and is registered with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Aboriginal Health Worker: Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care. Aboriginal Health Workers willing to obtain Cert IV in Aboriginal Primary Health Care (Practice) will be supported to transition to an Aboriginal Health Practitioner position upon completion and registration with AHPRA.
  • Eligible to drive in NSW and the ability to travel as required.

The Role

As an Aboriginal Health Practitioner, Primary Care, you will work as a key member of our multidisciplinary health centre team to provide flexible, holistic, and culturally sensitive health care services to Aboriginal and/or Torres Strait Islander people and apply key strategies for face-to-face patient care.

There is a focus on prevention and management of chronic disease for Aboriginal clients and supporting them on their journey throughout custody back into community, particularly those with chronic or complex health conditions.

You will work collaboratively with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations and other relevant health and community groups to ensure the delivery of seamless, timely and effective health care to Aboriginal clients.
