Subcontractor Support Coordinator

Location: Logan / Beenleigh Region (Hybrid – Office, Community & Remote Work)

We are currently seeking an experienced Support Coordinator (Level 2 ) to join our growing team on a subcontractor basis. This hybrid role offers the flexibility to manage your own schedule while maintaining some face-to-face meetings with participants, families, and service providers.

About the Role

As a Support Coordinator with 3 Minds QLD, you'll work collaboratively to:

• Assist participants to understand and implement their NDIS plans.
• Build participants' capacity to exercise choice and control.
• Coordinate mainstream, informal, and funded supports.
• Develop, monitor, and review participant goals and progress.
• Maintain accurate, timely documentation in line with NDIS Practice Standards.
• Engage in continuous professional development and industry updates.

This is a hybrid and flexible position where you manage your own workload, hours, and expenditure as an Independent Contractor.

About You

You are:

• Proactive, resilient, and adaptable with strong time-management skills.
• A confident communicator with excellent verbal and written abilities.
• Compassionate, ethical, and passionate about empowering people with disabilities.
• Comfortable working autonomously and collaboratively in a small, values-driven team.
• Experienced in delivering Level 2 and/or Level 3 Support Coordination services.
• Confident supporting participants with complex needs (highly regarded).

Requirements

To be successful in this role, you will need:

• 100 points of identification.
• Current Driver's Licence and comprehensively insured vehicle.
• Full Australian working rights.
• ABN (Australian Business Number).
• NDIS Worker Screening Check (current).
• Working with Children Check (relevant to your state).
• Public Liability and Professional Indemnity Insurance.
• Relevant qualification(s) in community services, social work, or similar.
• Demonstrated commitment to person-centred practice and a can-do attitude.

Pay: $61,165.48 – $102,681.94 per year
