Wise Choice Home Care is a community based aged care and disability service provider – supporting clients and families across Western Sydney.

Our mission is to deliver excellence in personalised care solutions and services, so our clients and families can live in the comfort of their own homes as independently as possible, for as long as possible.

We are currently seeking to recruit a compassionate, dedicated, and motivated Case Manager to join our fast-paced operations team.

Roles and Responsibilities:

  • Work with a caseload of clients
  • Liaise with clients/families as required to ensure requested services are being delivered
  • Manage client funding to ensure care needs are delivered within budgeted means.
  • Undertake assessments of clients.
  • Developing and reviewing care plans for individually funded clients
  • Coordinate, implement and review customised support services for clients.
  • Collaborate with health professionals to achieve client goals.

Essential

  • Experience in similar role in community or aged care setting.
  • Knowledge of the common physical, psychological and social issues affecting frail older people and people living with dementia
  • Highly developed interpersonal and communication skills, including ability to undertake complex negotiations and develop networks, relationships and partnerships with internal and external stakeholders
  • Proven high level organisational, leadership and motivational skills
  • Demonstrate high standards with the desire to continuously improve
  • Must have a Driver License

Desirable

  • Arabic, Assyrian or Vietnamese Speaking
  • Demonstrated understanding of the Australian Government funded aged care system.
  • Experience or understanding of NDIS/Aged Care
  • Experience in conducting competency assessments for care workers
