About the role

Join the team at Plan Wise Living as a Support Coordinator (NDIS) in Oran Park, NSW. In this part-time role (with the option to increase to full-time), you will play a crucial part in working with individuals with disabilities to access the services and support they need to live independently and achieve their goals.

What you'll be doing

  • Develop and implement individualised support plans to meet the unique needs of NDIS participants
  • Coordinate the provision of services from a range of providers, ensuring seamless delivery and high-quality care
  • Advocate on behalf of participants to secure appropriate funding and resources from the NDIS
  • Build and maintain strong relationships with participants, their families, and a network of service providers
  • Monitor and review participants' progress, adjusting support plans as needed
  • Provide guidance and assistance to help participants navigate the NDIS system
  • Maintain detailed records and documentation in line with NDIS and organisational requirements

What we're looking for

  • Minimum 2 years' experience in a similar support coordination or case management role, preferably within the disability or aged care sector
  • Demonstrated understanding of the NDIS and its participant-centred approach
  • Understanding of the role of Public Trustee and Guardian (TAG), developing and maintaining a strong rapport
  • Understand and work within the Mental health sectors, including Health, Justice and Correction Services
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse stakeholders
  • Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for participants
  • Proficient in using technology and relevant software to maintain accurate records and reporting
  • Preferred (but not essential) Relevant tertiary qualification in a human services or allied health discipline (e.g., social work, occupational therapy, psychology)
  • Willingness to travel to meet with participants in their homes or community settings
