Your new role:
As a key member of our team, you'll provide clinical primary health care services tailored to the unique needs of the Aboriginal and Torres Strait Islander community. Your role will involve case management, care coordination, and direct support for individuals dealing with mental health, substance use, trauma, grief, and social issues. In partnership with other healthcare professionals, you will help navigate the system, provide health education, and advocate for cultural competence across the board. Key Responsibilities:
- Deliver primary healthcare within a multidisciplinary team, in line with healthcare plans and scope of practice.
- Manage a diverse caseload, focusing on mental health, substance use, trauma, and social issues.
- Engage effectively with Aboriginal and Torres Strait Islander individuals and families.
- Navigate clients through the healthcare system and ensure continuity of care.
This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.

We are looking for someone with a genuine commitment to Aboriginal and Torres Strait Islander health, who can demonstrate cultural competence and a strong understanding of community needs. You should have the skills to work collaboratively in a multidisciplinary team and communicate effectively with diverse groups. A passion for making a positive difference in the lives of individuals and families is essential. Essential Skills & Experience:
- Demonstrated cultural knowledge of local Aboriginal and Torres Strait Islander communities.
- Experience working within Aboriginal and Torres Strait Islander Primary Health Care practices.
- Strong communication skills and the ability to engage with diverse community groups.
- Understanding of health literacy and the social determinants of health.
