We are currently seeking an Intake Assessment Officer for our home care packages program. The successful applicant will be responsible for providing support to clients accessing the support at home program with care planning, service coordination and monitoring of client care packages.
To be successful in this role you will needQualifications
Essential
- Degree or similar qualifications in nursing, social science, allied health, case management or related areas.
- Current drivers licence
- Employee working with children check
Essential
Experience working in home and community services / aged care services including review and evaluation of needs.
DesirableHigh level of computer literacy including Microsoft Office suite and experience in CareLink+ would be advantageous
Key selection criteria 1Demonstrated high level of communication skills both verbal and written with a wide variety of people.
Key selection criteria 2Demonstrated knowledge of the Strengthened Aged Care Quality Standards, the New Aged Care Act 2024, Aged Care Code of Conduct and statement of rights.
Key selection criteria 3Demonstrated ability to build productive relationships with a range of key internal and external stake holders.
Key selection criteria 4Ability to use initiative to problem solve and make informed decisions when required.
