About us

We are looking for a Support Coordinator to join our team at Bethel Lifecare. Bethel Lifecare is registered as a provider for the National Disability Insurance Scheme (NDIS) and offers a range of disability services in NSW. Our supports are tailored to assist participants live safely and comfortably at home and to actively enjoy their local community. Our mission is to promote and enhance the quality of life of participants through a client-centric approach. This will be achieved in a positive environment which fosters respect, empathy, independence, dignity, companionship and partnership. We strive to deliver high quality services for each participant using a person-centred approach and continuous improvement activities.

Qualifications & Experience

  • NDIS Workers Screening Check and Working with children's check (or willing to obtain)
  • Current drivers' licence
  • National Criminal Check
  • Knowledge and understanding of NDIS
  • Previous experience in the sector working with participants
  • Excellent verbal and written communication skills
  • Understanding of person-centred care
  • Strong advocacy and networking skills

Tasks & Responsibilities

  • Helping participants prepare for their plan review meetings or change of circumstances as needed or required, attending plan review meetings as booked, planned.
  • To ensure there are no identified conflicts of interest between service providers and any internal operations, like service work to participants
  • Keeping open, clear and positive relationships with participants, families and carers, service providers, NDIA Hotline and planners, Social Futures Local Area Coordinators, The NDIS Quality and Safeguards Commission, other relevant government agencies
  • For all supports provided to NDIS participants, ensure the correct billable hours are maintained, concise accurate files, ,maintain consistent progress reports for all scheduled plan reviews and reports are completed for the NDIS notes for evidence of all work and hours charged
  • Along with participants, help develop action plans which enable them to achieve their NDIS goals; skills; and informal and mainstream supports as well as submit relevant documentation to the NDIA for plan reviews, applications for nominee status, change of circumstances
  • Link and work with service providers, knowing the available services to enable choice and control for the participants and support participants in their process for informed decisions with service agreements from providers in an open and clear conversation with all contributors
  • Helping participants and carers with how to set up their myGov and access, showing them how to use their Proda portal
  • Oversee the participants funds through the Proda portal, ensuring best use of their budgets in the pursuit of them achieving their goals
  • Read, understand and comply with all WHS related policies and procedures, work related activities are to be conducted in a safe and proactive WHS manner, continued input and consultation for safety in the workplace. Follow NDIS Policies and Procedures
