Step into a pivotal leadership role with Darling Downs Health in Toowoomba as a Clinical Nurse Consultant – Clinical Coordinator, where your expertise will guide the delivery of high-impact, person-centred care through the Transition Care Program. You'll champion recovery, inspire a multidisciplinary team, and help reimagine community-based rehabilitation with compassion, clinical excellence, and innovation at its core.

What your day will look like….

You'll lead the coordination of care pathways within a dynamic multidisciplinary team, delivering short-term, high-impact rehabilitation services that support individuals during critical transitions in their recovery. Your role combines advanced clinical decision-making, collaborative stakeholder engagement, and professional mentoring to ensure seamless, person-centred care. Each day, you'll champion quality improvement, enhance service protocols, and inspire your team to achieve exceptional health outcomes.
