Community Living Facilitator

We're looking for a Community Living Facilitator who is passionate about inclusion, empowerment, and highly individualised practice.
In this dynamic and relationship-focused role, you'll support people with disability and their families to design, coordinate, and manage services that enable them to live full, connected lives.
This is a 10-month, 0.8 FTE position (Tuesday–Friday) based in Christies Beach and offers flexible work-from-home options.
Key Responsibilities
- Partner with individuals and families to design and coordinate individualised support services
- In collaboration with people and their families, recruit, orient, and supervise support workers
- Coordinate complex care plans and quality compliant service delivery.
- Build and maintain trusted relationships with people, families, colleagues, and external partners (e.g. Support Coordinators, Plan Managers, allied health professionals).
- Prepare documentation, reports, and communications with accuracy and timeliness.
- Apply understanding of NDIS funding arrangements in service planning.
- Contribute to a positive, collaborative, and values-driven team culture.
- Occasionally work after-hours and on weekends when required (TOIL provided).
- Participate in after-hours on-call assistance roster (approximately one week, every six weeks), with an allowance provided.

Essential Requirements
- Current First Aid Certificate
- Current NDIS Worker Clearance
- Current Driver's Licence and access to a reliable car for work purposes
- Availability for after-hours assistance roster
- Willingness to travel across Adelaide (Gawler, Hills, southern suburbs, city)
Desirable Background
This role would suit someone with experience in:
- Allied Health, Support Coordination, Local Area Coordination, or Community Development
- Coordinating or managing complex support services
- Building capacity in support teams and families
