We're looking for a Community Living Facilitator who is passionate about inclusion, empowerment, and highly individualised practice.

In this dynamic and relationship-focused role, you'll support people with disability and their families to design, coordinate, and manage services that enable them to live full, connected lives.

This is a 10-month, 0.8 FTE position (Tuesday–Friday) based in Christies Beach and offers flexible work-from-home options.

Key Responsibilities

  • Partner with individuals and families to design and coordinate individualised support services
  • In collaboration with people and their families, recruit, orient, and supervise support workers
  • Coordinate complex care plans and quality compliant service delivery.
  • Build and maintain trusted relationships with people, families, colleagues, and external partners (e.g. Support Coordinators, Plan Managers, allied health professionals).
  • Prepare documentation, reports, and communications with accuracy and timeliness.
  • Apply understanding of NDIS funding arrangements in service planning.
  • Contribute to a positive, collaborative, and values-driven team culture.
  • Occasionally work after-hours and on weekends when required (TOIL provided).
  • Participate in after-hours on-call assistance roster (approximately one week, every six weeks), with an allowance provided.

Essential Requirements

  • Current First Aid Certificate
  • Current NDIS Worker Clearance
  • Current Driver's Licence and access to a reliable car for work purposes
  • Availability for after-hours assistance roster
  • Willingness to travel across Adelaide (Gawler, Hills, southern suburbs, city)

Desirable Background

This role would suit someone with experience in:

  • Allied Health, Support Coordination, Local Area Coordination, or Community Development
  • Coordinating or managing complex support services
  • Building capacity in support teams and families
