Our team is passionate about creating safe, inclusive, and empowering environments for the people we serve and the employees who make it happen.

About the role

As a Community Connector you'll be working directly with vulnerable older people facing difficult circumstances such as social isolation, homelessness, mental health issues, substance use, family and domestic violence, or justice system involvement. You'll build relationships with individuals in the community, listening to their concerns without judgement and supporting them to access aged care and essential services. • Support clients to interact with My Aged Care and relevant assessment services.
  • Promote ADA Link services by networking with other community organisations.
  • Maintain high standards of documentation and consultation.
  • Liaise with other care finder services and support groups for older people.
  • Forge strategic partnerships with diverse cultural and community groups.
