About Us?The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Early Intervention Projects consist of the Inter-generational Program (North and South) and Chronic Conditions and social isolation (Client Support Facilitator) aiming to identify and support Aboriginal and Torres Strait Islanders over 50.
About the Role?We have a full-time fixed term opportunity available with possibility to extend.
This role supports Aboriginal and Torres Strait Islander people aged 50 and over by coordinating access to appropriate health and social services. Working closely with IUIH teams and external partners, you will help connect Elders to the supports they need, addressing barriers and service gaps in a culturally safe and respectful way.
You will also assist in delivering the Inter-generational Program by building strong relationships with Elders and strengthening their connection to community and culture.
Within this role, you will be required to:
- Liaise, consult and facilitate referrals in collaboration with targeted Aboriginal and Torres Strait Islander health and aged care services to identify barriers to client's access and engagement.
- Assist in the planning, development and implementation of the programs for the clients using a variety of appropriate intervention models with a continuity of care approach.
- Support the assessment of client's needs, being the central contact for clients through a rolling caseload, ensuring priorities and actions are met in partnership with other internal and external stakeholders.
- Provide culturally appropriate assessment, interventions and facilitate referrals for clients identified to reduce and manage social isolation, loneliness and chronic conditions.
About You?
- Current C Class Drivers Licence (QLD)
- Ability to travel across Southeast Queensland (SEQ)
- Current Criminal History Check
- Current Working with Children Clearence (QLD Blue Card)
- Current Disability Worker Screening Check (QLD Yellow Card)
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
