Northern Adelaide Local Health Network (NALHN) is eager to recruit an experienced Social Worker to work in our dynamic, growing and supportive team, to help us deliver exceptional care to our community.
This role provides clinical services, leadership and coordination in conjunction with other health professionals to optimise patient functional and health outcomes in this specialty area. As a highly skilled and competent clinician, working as a trans-disciplinary team member to provide a 7 day service that will deliver intensive, wrap around, therapeutic intervention with a 'Return Home First' philosophy to support safe and sustainable patient discharge, while providing support and supervision to less experienced staff.
About the Role:
In this role, you'll work within a multidisciplinary team providing the development, implementation, coordination and evaluation of quality acute hospital intensive restorative therapy services for patients of the Northern Adelaide Local Health Network (NALHN).

This role reports to the Senior Manager of Social Work for professional and operational matters and participates as part of a dedicated Acute Medical Unit team, reporting day to day with the Allied Health Acute Medical Unit Coordinator.
As a Senior Social Worker, you will be involved in complex case coordination and provide clinical expertise to support inter-disciplinary assessment, care planning, service delivery, and discharge coordination for acute medical patients. A key focus is contributing to effective discharge planning aligned with the 'Return Home First' philosophy, ensuring patients/carers experience safe and timely transitions to their home environment.
In this fast-paced environment, you will deliver high-level, patient-carer centred care through comprehensive discipline and inter-disciplinary assessments and timely interventions that support recovery, prevent functional decline, and facilitate safe discharge, collaborating closely with medical, nursing, and allied health colleagues to optimise patient outcomes, reduce hospital admissions, and enhance service efficiency.
Working across 7 days and public holidays may be required.
To be eligible for this position, you must hold an appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
