Clinical Care Coordinator – Aged Care (Contract)

Location: Newcastle

Contract Duration: 6-8 Weeks

We are seeking an experienced Clinical Care Coordinator to step into a vital leadership role within a welcoming aged care home. This is a hands-on contract opportunity where you'll make an immediate impact, support the team, and ensure residents receive exceptional, person-centred care.

What you'll be doing:
  • Oversee clinical governance, compliance, and accreditation requirements
  • Collaborate with the Facility Manager and Care Manager to meet clinical KPIs
  • Conduct assessments, care planning, and reviews to ensure best practice
  • Support staff education and development in line with aged care standards
What you'll bring:
  • Current AHPRA registration as a Registered Nurse
  • Demonstrated experience in a Clinical Care Coordinator / Care Manager role in aged care
  • Strong leadership and communication skills
  • Solid knowledge of aged care legislation, ACFI/AN-ACC, and clinical compliance
  • Ability to hit the ground running and adapt quickly in a fast-paced environment
