Newcastle Anglican is seeking a Care Partner to join our dedicated Community Aged Care team. This role is ideal for someone who is passionate about supporting older people to live independently and with dignity in their own homes.

About the Role
As a Care Partner, you will build trusted relationships with participants as you assess and coordinate support services in alignment with their package within Commonwealth Home Support Services (CHSP), Support at Home (SaH) or Home Care Packages (HCP). Collaborating closely with participants, their families and carers, Allied health and clinical teams, you will develop holistic care plans that reflect individual goals, strengths, and preferences to achieve their goals at home.
This is a non-clinical role focused on care coordination, participant engagement, and service excellence.
Key Responsibilities
- Coordinate care and support services in line with Aged Care Quality Standards.
- Develop goal-oriented care plans and budgets in consultation with participants and stakeholders.
- Provide education and resources to empower participant choice and control.
- Maintain accurate records in My Aged Care and other systems.
- Participate in service development initiatives to enhance wellbeing and quality of life.
- Collaborate with internal and external stakeholders to ensure integrated service delivery.
- Contribute to service excellence, continuous improvement and respond to aged care reforms.
Essential Requirements
- Experience working in aged care across community or residential, including participants with dementia.
- A solid understanding of CHSP, SaH or HCP funding
- A strong collaborator with excellent communication and interpersonal skills.
- Ability to work collaboratively within multidisciplinary teams.
- Excellent planning, time management, and organisational skills.
- Strong decision-making and problem-solving and adaptability
- Confidence using technology and contemporary systems.
- A commitment to wellness, safety, and continuous improvement.
- Certificate IV or Diploma in Community or Aged Care is desirable
- Current unrestricted driver's licence and access to a registered, insured vehicle.
- Current NDIS Worker Screening Check.
- National Police Check.
- Australian work rights.
