About the Role

Multiple Positions available

The Initial Assessment and Planning Worker will provide immediate and/or timely assessments and crisis intervention to Aboriginal People experiencing Homelessness. Both outreach and office-based IAP services will be provided by the Aboriginal Entry Point Team.

Using a collaborative and planned approach to ensure our clients get the services and supports they need to reach their goals. Using Culturally safe practises, the IAP Worker will assess client needs (and family where appropriate) and arrange, coordinate and advocate for the delivery and access to a range of programs and services that support the client to reduce the risk of homelessness and/or achieve stable housing.

Key Responsibilities

  • Respond promptly and appropriately to walk-in clients seeking support
  • Provide assertive outreach services to rough sleepers, hotspots and to community spaces
  • To respond to referrals within 24 hours by contacting the client or referrer
  • Conduct initial client assessments and deliver timely, responsive interventions and outcomes, while collaborating effectively within a team environment
  • Provide interim case management support to help community members become 'housing ready'
  • Maintain accurate and up-to-date client records and case notes in SHIP, ensuring clear, concise, and precise data entry
  • Maintain a high standard of professionalism including client confidentiality
  • Conduct client feedback and relay such feedback to your direct report
  • Promptly report concerns to management and contact appropriate emergency services when necessary
  • Be available to take up additional responsibility as directed by Team Leaders or Manager
  • Attend relevant internal and external meetings, required to attend VISHN conferences

Skills & Experience

  • Certificate IV in Community Services or relevant qualification (Desirable)
  • Experience in welfare work and / or community development
  • Experience in working with Aboriginal people experiencing or at risk of homelessness
  • Demonstrated understanding of the unique health challenges and social issues faced by the Aboriginal community, including the impacts of alcohol and drug use and homelessness.
  • Ability to provide culturally safe and respectful care and services to the Aboriginal community.
  • Leadership skills to lead and inspire a team, fostering a collaborative and supportive work environment.
  • Networking skills to build and maintain relationships with various stakeholders, including local health services, law enforcement agencies, and Aboriginal organisations.
  • Outreach skills to conduct assertive outreach within the community, demonstrating sensitivity and respect for cultural norms and practices.
  • Computer literacy skills including Microsoft Office suite

Mandatory Requirements

  • Current Victorian Driver's License
  • National Police Certificate (no older than 3 months).
  • Victorian Employment Working with Children Check
