About the Role

As an Integrated Family Services Practitioner, you'll play a key role in strengthening families, supporting safety and stability, and helping children and young people thrive. You'll partner with families experiencing vulnerability, using a strengths-based, trauma-informed approach to build capacity, resilience and positive change.

Working on a fixed term, part- time basis you will work across a range of programs, including Adolescent Support, Finding Solutions, Family Services, Family Preservation & Reunification, Targeted Care Packages, and Who's In Charge. In this role you will be offering practical support, early intervention and guidance tailored to each family's goals.

Collaboration is central to this role. You'll build strong relationships with schools, health services, community agencies and other stakeholders to create coordinated support around families.

A full list of key duties is outlined in the Position Description, but includes responsibilities in:

  • Undertake case management including intake, assessment, case/care planning, case coordination, key worker role, brief interventions, supported referrals, action plans, case reviews and case closure.
  • Establish, facilitate and maintain action plans and outcome star plans for adults and families.
  • Work in close partnership with local housing offices, other housing and support providers, Gunditjmara Aboriginal Cooperative, Southwest Health Care – Mental Health Services and Western Region Alcohol and Drug Centre and local government and departmental-funded public tenant groups.
  • Establish appropriate community contacts and develop and maintain professional working relationships with a broad range of stakeholders.

Do your skills include:

  • A sound understanding of issues confronting disadvantaged children, young people and their families, particularly clients involved with the child protection system and out of home care program.
  • The ability to make timely, sound decisions and assessments regarding children, youth and families, particularly under pressure
  • Demonstrated knowledge and understanding of the appropriate legislation, case management practice, and the ability to adhere to policy and procedures.
  • The ability to maintain positive, effective working relationships with colleagues, peers, and relevant stakeholders in a wide range of government, business and community organisations.

Are your credentials:

  • A minimum qualification of a sector recognised and accredited Bachelor of Social Work or Diploma in Community Welfare Work or equivalent. (Qualifications must be approved by recognised professional bodies: ie. AASW, APS etc.)
  • A current Drivers Licence.
