About the role

Best Life Individual Support Services is seeking a dedicated Care Coordinator to join our team in Nambour, Queensland. As a Care Coordinator, you will play a vital role in ensuring our aged and disabled clients receive the highest quality of care and support to live their best life. What you'll be doing

  1. Coordinate and oversee the delivery of personalised care plans for our aged and disabled clients
  2. Liaise with clients, their families, and our care teams to ensure seamless service provision
  3. Monitor and evaluate client progress, adjusting care plans as needed
  4. Identify and address any issues or concerns to maintain the highest standards of care
  5. Collaborate with healthcare professionals and community service providers to coordinate comprehensive care
  6. Maintain detailed records and documentation in line with industry regulations
  7. Provide leadership, guidance, and support to care staff to foster a positive and productive work environment

What we're looking for

  1. Bachelor in Nursing or Psychology or equivalent qualifications in similar fields
  2. Minimum 4 years working in the NDIS sector
  3. Minimum 2 years' experience in a care coordination or case management role
  4. Strong understanding of disability support services and funding models
  5. Excellent communication and interpersonal skills, with the ability to liaise effectively with clients, families, and healthcare providers
  6. Proven problem-solving and decision-making skills, with a focus on client-centric care
  7. Proficient in using electronic case management systems and data tracking
  8. Empathetic, compassionate, and committed to making a positive difference in the lives of our clients
