Footprints Community is a well-regarded not-for-profit provider of community-based services, working in the community for over 30 years. We specialise in working with older people, those that experience disability, mental illness, as well as those who are at risk of homelessness.
About the Position
The Journey Coordinator plays a
pivotal role within the MyndKind service system, acting as an independent
partner in facilitating an individual?s care journey. This position may
function as a standalone service, be embedded within a Front Door service, or
complement other MyndKind offerings. The Journey Coordinator offers
person-centred practice, ensuring individuals are at the core of their health
and wellbeing. Through collaborative care coordination, the role aims to
enhance service integration, improve access to appropriate supports, and
promote positive mental health outcomes.

About You
- Proven experience in building partnerships and collaborations that improve participants outcomes and service delivery.
- Strong ability to develop and maintain genuine, respectful relationships with individuals accessing MyndKind.
- Demonstrated capacity to build rapport and trust through a recovery-focused, person-centred, strengths-based, and trauma-informed practice framework.
- Sound understanding of mental health challenges and their impact on individuals? experiences and support needs.
- Demonstrate professional and ethical practice.
- Advanced communication, interpersonal, and organisational skills.
- Understanding of mental health challenges and their impact on individuals.
- Knowledge of, or the ability to quickly acquire knowledge of, regional supports and services to enhance service navigation.
- Ability to work both collaboratively and independently.
- Have the ability to meet deadlines in a high demand work environment and to exercise judgment and initiative.
- Proficient in the use of Microsoft Office Suite and digital client management software.
- Holds a current Queensland C-Class Driver?s Licence.
- A Federal Police Check, Blue Card, and National Worker Screening or Yellow Card is are required for this role.
