We are currently seeking a Client Support Officer to join our team and help us continue making a difference in the lives of our Elders.
Position Overview
The Client Support Officer is a key member of the Client Support Team, responsible for coordinating and delivering personalised care to Wyanga clients. You will manage care plans, coordinate services, and be the primary contact for clients and their families, ensuring that their individual needs are met with professionalism and respect.
Key Responsibilities
Client Care Coordination:
Develop and manage personalised care plans, ensuring timely delivery of services such as personal care, domestic assistance, and social support.
Client Communication & Support:
Be the main point of contact for clients and families, offering guidance, support, and resolving any concerns.
Compliance & Documentation:
Ensure all client documentation is accurate, up-to-date, and compliant with aged care regulations and standards.
Team Collaboration:
Work closely with other team members to ensure seamless care delivery, contribute to meetings, and support the integration of new staff.
Continuous Improvement:
Identify areas for improvement in client services and participate in ongoing professional development.
Qualifications & Experience
- Certificate III in Individual Support or IV in Ageing Support, Community Services, or related field, or equivalent experience.
- Current Driver's Licence and reliable transport.
- Previous experience in a client care or case management role, preferably in aged care or community services.
- Strong knowledge of Aged Care Quality Standards and Home Care Packages (HCP) and CHSP funding models.
Skills
- Excellent organisational skills and the ability to manage a caseload effectively.
- Strong communication skills, with an empathetic and client-centered approach.
- Proficiency in client management systems, finance systems, HR systems, and Microsoft Office Suite (Word, Excel, Outlook).
- Ability to work both independently and as part of a collaborative team.
