Ballarat Hospice Care is seeking a compassionate
Supportive Care Advisor
to join our multidisciplinary team. In this rewarding role, you will provide emotional, psychosocial, spiritual, and bereavement support to patients and their families in their own homes, helping people live, die, and grieve well. You will conduct assessments, provide counselling, support care planning, and liaise with both internal teams and community services. Some planned out-of-hours activities may be required.
