As our invaluable Social Connector, you will play a vital role in empowering clients to lead fulfilling lives through Helping Hand's social connections program. You will create inviting spaces with engaging group activities that promote friendships and reduce isolation, as well as provide personalized, high-quality one-on-one support tailored to individual needs within the client's home or community.

You'll also make an impact by:

  • Collaboratively working with the Social Connections Lead to develop and refine social and wellbeing programs tailored to individual and group client needs.
  • Reporting any incidents, safety concerns, or changes related to your work, client services, or personal well-being through the appropriate systems.
  • Ensuring that Helping Hand electronic documentation is accurately completed and submitted within the required timeframes. This includes, but is not limited to, employment administration forms, incident report forms and leave forms.

What you'll bring

To succeed in this role, you will have:

  • Minimum Certificate III qualification gained in a relevant field of lifestyle co-ordination, disability, community care or aged care.
  • Demonstrated ability to work with limited supervision, exercise initiative and problem-solving skills
  • Strong customer service skills, professionalism, respect for client rights, and the ability to provide personalized, client-centered services.
  • Excellent communication, active listening, a positive outlook, teamwork skills, and clear written and verbal expression.
  • Possesses strong time management and organizational skills, working independently, adapting to changing priorities, and handling schedule changes with flexibility.
  • Valid NDIS Worker Screening clearance or be willing to obtain one.
  • Valid driver's license and own car or access to Vehicle

You'll stand out from the crowd if you have some of the following:

  • Certificate IV in Lifestyle & Leisure Activities
  • Experience facilitating social groups for older adults or people with disabilities is highly desirable
