About the Role
We are currently seeking a qualified and passionate Community Care Worker to join our growing team. Based at our Oakleigh office, you will play a key role in supporting NDIS participants to live fulfilling, independent lives by connecting them with the services, resources, and opportunities they need.
Key Responsibilities
As a Community Care Worker, you will be responsible for delivering high-quality, person-centred support to individuals with disabilities and aged care clients under the NDIS and Home Care Packages. Your duties will include:
- Assessing client needs through interviews and comprehensive intake assessments to understand the nature and extent of personal, emotional, social, and financial difficulties.
- Planning, developing, and implementing tailored support programs, including educational and training initiatives, that align with each client's NDIS goals and preferences.
- Monitoring and evaluating client progress, regularly reviewing support plans and adapting strategies as circumstances change. Provide accurate documentation and reports.
- Referring clients to appropriate community, health, housing, education, and employment services, and ensuring timely and effective access to additional support where needed.
- Assessing community needs and available resources, identifying gaps in local services, and contributing to the development of new initiatives that enhance community wellbeing.
- Liaising with community groups, government agencies, welfare organisations, and private providers to coordinate services, advocate for client needs, and raise awareness of available support options.
- Supporting families and providing direct care for children, elderly individuals, and persons with disabilities across various settings, including in-home care, adult service units, group housing, and supported accommodation.
- Preparing applications and submissions for NDIS funding, grants, and community resources, and reporting to government bodies and partner organisations as required.
- Supervising and supporting clients on parole or probation, where applicable, helping them reintegrate into the community with dignity and stability.
- Assisting youth with navigating social, emotional, and financial challenges, helping them build resilience and achieve greater independence.
- Organising and participating in outreach and community engagement events to promote inclusion, provide information, and support clients' participation in recreational, social, and cultural activities.
- Promoting independence and inclusion through skill development, daily living assistance, transport support, and community participation in line with NDIS and aged care plans.
- Promote inclusion and participation through community-based activities and events.
Required Qualifications & Experience
- A Diploma or higher in Community Services, Disability, Social Welfare, or a related field.
- Minimum 1 year of relevant experience in community care, disability support, or social services.
- Strong understanding of NDIS standards, frameworks, and participant rights.
- Excellent interpersonal, communication, and advocacy skills.
- Ability to work autonomously and within a multidisciplinary team.
- Must hold a valid Driver's Licence, First Aid & CPR certification, and have current Police Check and WWCC (Working With Children Check).
- Experience in case management, care planning, or community development is highly regarded.
