The opportunity
We're excited to offer a rewarding opportunity at Diversicare, part of the Ethnic Communities Council of Queensland (ECCQ), for an experienced Care Partner to support our Home Care Package (HCP) clients.
This full-time role is based on our Brisbane office (Indooroopilly), easily accessible via public transport and close to local amenities.
As a Care Partner, you'll play a key role in empowering clients to live independently and with dignity in their own homes. You'll be part of a passionate team of over 120 staff committed to culturally inclusive, person-centred care.
What We're Looking For
- Proven experience in Home Care services (HCP)
- Strong interpersonal and coordination skills
- Solid understanding of Aged Care and Community Care Quality Standards
- Strong verbal and written communication skills, with the ability to influence and build rapport
- A proactive mindset with problem-solving skills and keen attention to detail
- Cultural awareness and sensitivity in a diverse workplace environment
- A customer-focused approach that puts client wellbeing at the centre of everything you do
Key Responsibilities
- Reporting to the Care Partner Team Leader you will
- Build strong relationships with clients and their families through regular check-ins to ensure satisfaction and address any concerns
- Conduct comprehensive in-home assessments and develop tailored care plans that support independence and wellbeing
- Collaborate with internal teams and external providers to ensure seamless, culturally responsive service delivery
- Monitor client needs and adjust services proactively to reflect changes in health or circumstances
- Maintain accurate and up-to-date client records in line with compliance and quality standards
- Generate regular reports to support management oversight and continuous improvement
- Manage Home Care Package (HCP) budgets to align with client goals and care priorities
- Support clients transitioning into care packages and assist with smooth transitions into residential care when needed
