About the role
The Clinical Nurse Consultant is a senior Registered Nurse who will provide clinical nursing leadership and operationally be responsible for supporting the First Nations Holistic Wellbeing Service – Murrumba Targan Djimbulung. The position is accountable/responsible for the initial planning and implementation of our model of care and to work collaboratively/in consultation, to evaluate this new model of care to align with the West Moreton strategic plan and Department of Healthcare Purchasing and System Performance Branch performance outcomes.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrate ability to provide culturally safe nursing care.
- Contemporary clinical knowledge in the areas of primary health care and /or chronic disease management, in a community setting in collaboration with individuals, peers and the healthcare team to achieve improved health outcomes across Preventative Health Services for Aboriginal and Torres Strait Islander people.
- Leadership in the coordination of nursing and provision of health services across the continuum of care, delivering competent nursing care in complex situations and ability to coach patients to adopt self-management
- Ability to adopt digital technology and utilise digital care solutions and telehealth to provide a coordinated approach to delivering care closer to
- Advanced clinical skills in the identification and adoption of innovative clinical practice models and work practices that develop partnerships to support the continuum of care for Aboriginal and Torres Strait Islander people including contemporary technologies (IT, ICT).
- Advanced level delivery of interpersonal (problem solving/conflict resolution), communication (verbal/written) and time management skills whilst demonstrating autonomous, sound, high level conceptual and critical thinking capabilities with and for Aboriginal and Torres Strait Islander people.
- Demonstrated expert level of clinical knowledge, expertise and skills in complex contemporary clinical practice, and delivery of evidence-based holistic health and wellbeing programs, in relation to Aboriginal and Torres Strait Islander people
- Ability to lead and maintain culturally appropriate aboriginal and Torres strait islander services.
- High level knowledge of barriers Aboriginal and Torres Strait Islander people face in accessing health care and the impact of health outcomes.
- High level knowledge of relevant Aboriginal and Torres Strait Islander policy frameworks.
