We are looking for a compassionate and highly organized Disability Support Service Coordinator to join our team and make a meaningful impact in the lives of people with disability. This role is central to ensuring that participants receive the right supports—tailored to their goals, preferences, and needs—under the NDIS framework.
As a Service Coordinator, you will work closely with participants, their families, support workers, and external providers to develop, implement, and monitor individualized support plans. You'll ensure services such as personal care, community access, appointments, and social activities are delivered smoothly and effectively.
This is a person-centered role that goes beyond scheduling—it's about building trust, empowering participants, and advocating for their choice and control. You'll conduct regular service reviews, maintain accurate case notes and documentation, and ensure all supports are aligned with NDIS goals and funding.
We're looking for someone who values dignity, inclusion, and empowerment, and who thrives in a collaborative, fast-paced environment. Your ability to problem-solve, communicate with empathy, and coordinate multiple services will be key to ensuring participants feel supported, heard, and respected in every interaction.

Key Responsibilities:
Match clients with suitable support workers based on skills, preferences, and compatibility
Maintain regular communication with clients, families, and care staff
Monitor and update client care plans to reflect changing needs
Ensure compliance with NDIS and Quality Standards
Liaising with support coordinators or case managers and customers to ensure services are provided in accordance with the customer's assessed needs and support professional's availability.
Deliver exceptional customer service and maintain accurate records
Conduct client assessments and develop tailored support plans
Monitor service delivery to ensure quality, compliance, and continuous improvement
Build and maintain strong relationships with clients, families, and local service providers
Maintain accurate and timely client records in line with funding and organizational requirements.
Assisting in the recruitment and management of community support staff
Participate in community engagement and outreach initiatives
What We're Looking For :
Minimum 1 years' experience as a Care Co-Ordinator
Strong understanding of NDIS Quality Standards (preferred)
Excellent organizational, time management, and communication skills
Compassionate and client-focused mindset
Proficiency in scheduling software and general IT systems
Relevant qualifications:
Certificate III or IV in Individual Support (Disability) or Diploma in Community Services
