We are seeking a seasoned Social Worker or Aged Care Specialist to partner with our General Manager to ensure our retired Brothers access the maximum supports available through My Aged Care - quickly and smoothly. If you know the system inside-out and love cutting through delays to deliver real outcomes, we'd love to hear from you.

What you'll do
  • Audit current supports for each Brother and identify gaps, entitlements and upgrade opportunities (HCP levels, CHSP, STRC, residential options, respite, supplements as applicable).
  • Fast-track My Aged Care: make/triage referrals, coordinate ACAT/RAS assessments, pursue priority reviews, manage waitlists, and secure interim services.
  • Build individual care plans and coordinate providers (home care, allied health, clinical services), including service changes and package upgrades.
  • Navigate the My Aged Care Portal on behalf of the person (representative arrangements), handle documentation, consents, and correspondence.
  • Work closely with GPs, assessors, hospitals, and providers to resolve blockers and expedite starts.
  • Provide clear reporting on progress, risks, and outcomes to leadership.
  • Travel to visit Brothers as needed (primarily NSW/VIC/QLD).
What you'll bring
  • Significant, hands-on experience with My Aged Care, including ACAT/RAS processes, HCP activation/upgrade pathways, CHSP bridging, and provider onboarding.
  • Tertiary qualifications in Social Work, Nursing, Allied Health, Gerontology, or similar.
  • Demonstrated success accelerating access to services and optimising entitlements for older Australians.
  • Skilled case coordination, advocacy, and stakeholder management (clinicians, assessors, providers, families).
  • High professional standards: confidentiality, dignity, cultural and faith awareness; trauma-informed practice.
  • Strong documentation/reporting skills; comfort working autonomously and to deadlines.
  • Current driver's licence; ability to travel.
  • National Police Check (or willingness to obtain) and current influenza/COVID-19 vaccination status suitable for visiting aged care settings.
  • Full Australian working rights.
