About The Role
This care coordinator role will be embedded into the Mental health AOD Hub team and report directly to the Team leaders. This position will also have strong links to the existing ED care coordination team, ALERT. Through screening and referral in the ED the care coordinator is responsible for the delivery of timely and comprehensive assessment and care coordination of patients with complex needs to facilitate patient centred care, effective management of the ED episode of care and support discharge planning and support into the community Assessment and care planning are achieved in collaboration with patients, their carer's, and members of the MH&AOD Hub team, ALERT and the Emergency Department Clinical team.
Classifications for this position will be Allied Health Grade 2 or Registered Nurse Grade 4A.

About The Emergency Department
The SVHM Emergency Department (ED) consists of 2 resuscitation bays, 16 cubicles, 5 fast track chairs and 15 Emergency short stay beds. In addition, there is a behavioural assessment room, procedure room and a plaster room. A triage assessment chair and a 3 chair diagnostic lounge supports patient flow into the department.
The Emergency Department has in place a multidisciplinary team which brings together clinicians from mental health, drug and alcohol and the ED care coordination team ALERT to provide specialised care to patients presenting to the ED with high acuity mental health and AOD issues, and complex psychosocial needs. This service is called the Mental Health and AOD Hub (MH&AOD Hub) and will be based at within the Emergency Department. All positions will report clinically to the MH&AOD Hub team leader, while direct line management sits within the retrospective discipline streams in the case of the care coordination role this will be through ALERT. The team will provide a seven day/week extended hours service.

POSITION PURPOSE
This care coordinator role will be embedded into the Mental health AOD Hub team and report directly to the Team leaders. This position will also have strong links to the existing ED care coordination team, ALERT. At times when not engaged in the care of MH&AOD Hub patients, this role will take on similar duties within the ALERT team.
POSITION DUTIES
MH &AOD Hub Role
- Respond and screen eligible clients for the MH&AOD hub
- Provide clinical response to eligible MH&AOD Hub patients
- Provide appropriate discipline specific clinical management, discharge planning & community liaison for all relevant patients in collaboration with team members within the Hub
- Effective negotiation and advocacy with relevant service providers to ensure optimal and coordinated client care according to best practice principles
- Facilitate appropriate admissions/nominations to, inpatient and outreach services.
- Provide advice and support and engage in strategies to maintain a safe and therapeutic environment for consumers and carers.
- Facilitate appropriate admissions/nominations to area mental health and AOD services, inpatient and outreach services.
- Consultation and follow up with community teams to ensure continuity of care.
- Ensuring all documentation including client records are completed and managed in a timely manner consistent with SVHM policies and procedures, legislative and regulatory requirements
- Contributing and participating in MH&AOD Hub, professional development (PD) and other PD that supports own learning objectives for this role.
- Active participation in MH&AOD Hub interface processes, service development, evaluation and portfolio activities
- Develop knowledge of local resources and services to make appropriate referrals, provide advocacy and initiate service development that will proactively enhance consumer access to services.
ALERT Role
When not directly involved with the MH&AOD Hub:
- Early identification of ALERT patients in the Emergency Department.
- Facilitate clinical management, discharge planning & community liaison for all relevant patients under the ALERT target cohort.

SELECTION CRITERIA
Essential Registration, License Or Qualification Requirements
- Nursing qualification or Allied health qualification relevant to the cohort and minimum 2 years' experience in relevant acute setting.
