We are seeking a hands-on, caring, and passionate Care Manager (Grade 5 RN) to lead our Memory Support Unit at Whitewater, Mordialloc

What You'll Be Doing

As a Care Manager, you will provide exceptional dementia care, working closely with a high-performing team to ensure a safe and supportive environment for residents. Your key responsibilities include:

  • Delivering comprehensive, person-centred nursing care

  • Developing recommendation plans & behaviour support strategies

  • Managing medication administration & wound care

  • Driving continuous clinical improvement for excellence in care

  • Supporting residents, families, and visitors with professionalism

  • Ensuring timely assessments, care plans, and reviews

  • Leading clinical education & staff competency training

  • Fostering a positive and collaborative workplace culture

What Matters to Us

We're looking for a dedicated professional with:

  • Minimum two years' experience in a clinical leadership role

  • Strong understanding of Aged Care Quality Standards

  • Division 1 Nursing qualifications & AHPRA registration

  • Up-to-date COVID-19 & 2025 influenza vaccinations

  • NDIS Check (incl. Police Clearance)

  • Eligibility to work in Australia
