As our family of Chinese-speaking clients continues to grow, we are seeking an experienced and empathetic Care Coordinator who can bridge language and cultural gaps to deliver truly personalised support.
Please note: We are also open to receiving applications from less experienced candidates for an Assistant Care Coordinator position, with a clear pathway for growth.

The Opportunity:
This is more than just a coordination role; it's an opportunity to become a trusted pillar for the Chinese-speaking community. You will be the vital link between our clients, their families, and their support workers, ensuring they feel heard, understood, and valued. You will use your language skills and cultural understanding to design care plans that respect individual preferences and traditions, making a profound difference in the lives of seniors and individuals with disabilities.
This is a full-time position based on a regular 9:00 AM to 5:00 PM schedule, Monday to Friday, offering a healthy work-life balance.
Key Responsibilities:
- Client-Centred Coordination: Manage and coordinate a caseload of clients eligible for Home Care Packages and NDIS services, ensuring a consumer-directed care approach.
- Care Plan Development: Conduct holistic assessments and create, implement, and review comprehensive, individualised care plans that align with client goals and preferences.
- Case Management & Rostering: Efficiently roster support workers to meet client needs, managing schedules, timesheets, and service delivery.
- Relationship Building: Build strong, trusting relationships with clients, their families, and support workers to ensure seamless communication and exceptional service delivery.
- Cultural Advocacy: Act as a cultural liaison, ensuring services are delivered in a culturally sensitive manner and promoting our services within the Chinese-speaking community.
- Administrative Excellence: Maintain accurate client records, prepare timely reports, and manage documentation in compliance with industry standards.
Qualifications & Experience:
For the Care Coordinator Role:
- Diploma in Community Services, Nursing, or a similar health/social care field.
- Minimum of 2 years of experience in a client service coordination role within the aged care, disability, or community services sector.
- Fluency in both English and Mandarin or Cantonese.
- Demonstrated knowledge of the Home Care Package (HCP) program or the new Support at Home (SaH) program.
- Current QLD Open Driver Licence and a reliable vehicle.
- Current National Police Certificate.
- Full COVID-19 Vaccination Certificate.
