Ray Foundation Group is a registered NDIS provider dedicated to empowering individuals with disabilities to live independently and with dignity. We provide high-quality, person-centred supports across Perth, with a focus on compassion, professionalism, and community inclusion.

We are seeking dedicated Disability Support Workers to join our growing team and support participants in achieving their goals.

Key Responsibilities

  • Provide personal care, domestic assistance, and community access supports as per participant care plans.
  • Assist with daily living tasks (meals, hygiene, mobility, medication prompts).
  • Facilitate social, recreational, and community participation activities.
  • Document progress notes and maintain accurate records in line with NDIS requirements.
  • Work collaboratively with participants, families, and other professionals to deliver individualised care.
  • Promote dignity, independence, and choice for all participants.

  • Ability to work independently and as part of a team.
  • Reliable, punctual, and professional.
  • Problem-solving skills and flexibility to adapt to participant needs.

Mandatory Requirements

  • NDIS Worker Screening Check (or willingness to obtain).
  • Working With Children Check (if supporting children).
  • First Aid & CPR Certificate (HLTAID011 / HLTAID009).
  • Manual Handling training (preferred).
  • Valid Australian Driver's Licence & access to reliable vehicle (desirable).
  • Proof of full working rights in Australia.

Desirable Qualifications

  • Certificate III or IV in Disability, Individual Support, Aged Care, or Community Services.
  • Experience working within the NDIS sector.
  • Bilingual skills (advantageous in multicultural communities).
