The Role

Silverchain has an exciting opportunity for a Client Service Co-ordinator to join our teams within the South service centreson a permanent full time basis. The working hours are 7am - 3pm.

The Client Service Co-ordinator will facilitate the effective and efficient entry and onboarding of new clients to Silverchain. Ensuring that all assessed care needs and approved Support at Home funding is discussed with clients, to highlight the comprehensive service delivery from Silverchain, which can support clients to achieve their goals and deliver the best outcomes for the client, their carers' and families.

Key Responsibilities

  • Progress all client referrals (both from existing clients who are transitioning across programs and for new external referrals).
  • Ensuring to maximise the conversion rate of referral to effective entry and onboard to Silverchain Support at Home services.
  • Undertake required documentation following the required procedures to efficiently and successfully onboard clients to Silverchain Support at Home services.
  • Work in partnership with the Acquisitions team to ensure all new and existing clients have been set up correctly for Support at Home funding program and pathways.
  • Communicate (actively listen, clarify, interpret, engage) with clients, carers, support persons and decision makers to ensure that client needs and preferences are understood and the value which Silverchains comprehensive services can bring to remain independent and cared for in their home.
  • Influence choices around complex care management with the support of multiple disciplines to ensure Best Care is delivered.
  • Work in partnership with the Acquisitions team to ensure all new clients have been set up correctly for Support at Home funding program and pathways.
  • Confidently and accurately present, promote and invigorate key Silverchain messaging of our strong position to deliver the best Health and Aged Care services in the home and community.
  • Identify at risk clients and potential risks to service delivery and consult with relevant team members, including facilitation of MDT meetings, prior to the establishment of a Support at Home Service Agreement
  • Deliver agreed KPIs to meet organisation and business unit objectives.
  • Document, report and action hazards and incidents to ensure safe work procedures are followed.

About You

  • Tertiary qualification or Certificate IV or Diploma in Aged Care Work; Disability Work or Community Services (Service Co-ordination) to ensure service delivery and outcomes.
  • Demonstrated experience in onboarding new clients and creating/developing/designing effective and sustainable care packages.
  • A demonstrated understanding of aged, community, disability and health care and the various applicable programs (e.g. Veterans' Home Care; Home and Community Care; Disability Services and Care Packages) to support service delivery within a competitive and growing industry.
  • Demonstrated ability to negotiate with and influence clients toward pathways that deliver quality client outcomes and experience and supports the client's independence and capability.
  • Demonstrated flexible approach to care delivery to facilitate positive and safe outcomes for client careers' and Silverchain staff.
  • A commitment to excellence in customer service to ensure the consistent provision of exceptional service to internal and external stakeholders.
  • Well-developed communication (written, verbal and listening) and interpersonal skills to develop effective relationships and facilitate employee communications.
  • Demonstrated ability to work autonomously and as part of a team always with a team focus to participate and contribute to the success of service centre and organisational objectives and vision.
  • Demonstrated time management and organisational skills to effectively plan and prioritise workflow.
  • Demonstrated computer literacy in word processing, spreadsheets, and presentation software to prepare effective correspondence and documentation.
