The Palliative Care Social Worker Level 4 – Clinical Lead, Carer and Bereavement Programs is a senior specialist role responsible for leading the design, implementation, and evaluation of carer and bereavement programs across ISLHD Community Palliative Care. With advanced expertise in grief, loss, and carer support, this position ensures high-quality, person-centred care for individuals and families affected by life-limiting illness and bereavement.
