We are currently seeking an experienced Mental Health Support Worker on Sunshine Coast with a strong background in Supporting individuals with Psychosocial Disabilities to join our team. This is a fantastic opportunity for someone who is passionate about supporting individuals with complex mental health and disability needs to achieve their goals and lead fulfilling lives.
Key Responsibilities:
- Provide tailored, person-centered support in home and in the community.
- Assist with Domestic and Household duties
- Assist with Shopping and Community access
- Assist with recreational activities.
- Medication prompting ( when required)
- Develop, Implement, and review NDIS support plans that are focused on achieving participants' goals..
- Monitor and review progress regularly to ensure the best outcomes for participants.
- Complete shift notes, incident reports, behaviours charts in the timely and professional manner.
Key Requirements:
- Strong communication and interpersonal skills with the ability to build rapport with participants and stakeholders.
- Ability to work independently and be part of the team.
- Compassionate and empathetic approach to working with people with complex needs.
- Relevant qualifications ( Cert 3 in Disability or Age Care) .
- Current NDIS Clearance and National Police Check.
Desirable:
- Previous experience working with individuals who have complex psychosocial needs.
- Knowledge and understanding of the local community resources available on the Sunshine coast and surroundings.
