Position Overview:
Rembrandt Living is experiencing an exciting period of growth and to compliment this we are seeking compassionate and skilled Home Care Service Coordinators to join our home care team located in Victor Harbor.
As a Service Coordinator with Rembrandt Living, you will prioritise client and customer satisfaction by engaging with families, conducting in-home Care Assessments and creating personalised care plans tailored to individual client needs. Our ideal candidates will have a strong background in providing high-quality care to clients in their own homes, demonstrate a deep understanding of individual care needs and have a commitment to patient-centered care.
Key Responsibilities:
- Promote Rembrandt Living Home Care as a preferred provider of service delivery choice for consumers and participants.
- Support quality assessment, care planning and service delivery processes, consistent with evidence-based practice and positive case management principles within the Governance Framework.
- Coordinate consumer services in line with funding agreements, individual budget requirements and individual goals.
- Coordinate consumer scheduling and staff allocation to promote continuity of service provision.
- Coordinate service delivery capacity to support people with high level, complex health care needs and optimise independent living at home and in the community.
- Maintain active oversight of data collection, client files and information management systems to ensure these are maintained in line with contract, reporting, privacy and confidentiality requirements.
- Attend regular staff meetings and participate in forums, reference groups and network meetings as required
- Comply with Rembrandt Living's policies and procedures.
About you
- Full time availability Monday - Friday
- A minimum of Certificate IV in Community Services
- A minimum of 1 year experience in coordinating community care or aged care services.
- Great attention to detail, communication skills and are confident using computer systems
- Strong organisational and time management stills
- Strong attention to detail with analytical and problem-solving skills
- A Current police clearance
- A Current driver's licence
