We provide high- quality services to paediatric and adult clients across Australia, offering support under the NDIS, Medicare, private health insurance, TAC, and WorkSafe. Our services include Speech Pathology, Occupational Therapy, Physiotherapy, Psychology, Dietetics, Music Therapy, Art Therapy, Social Work, Counselling, Audiology, Allied Health Assistance, Developmental Education, Exercise Physiology, Plan Management, NDIS Funding Application Assistance, Support Coordination, Household Tasks, Positive Behaviour Support and Support Worker Service.

We are currently seeking an enthusiastic and motivated Specialist Support Coordinator to join our growing team. This role is a subcontractor position, with flexible working hours and opportunities for professional growth.

About the Role

As a Specialist Support Coordinator, you will play a vital role in delivering high-quality, person-centred support to NDIS participants with complex needs. You will work collaboratively with individuals, their families, support providers, and broader networks to overcome barriers and ensure effective coordination of supports and services.

Key Responsibilities:

• Collaborate with NDIS participants and their support networks to identify goals, develop tailored support plans, and link participants with suitable providers to achieve positive outcomes.

• Foster participant engagement by building trust and encouraging active involvement in decision-making in their own support planning and implementation.

• Manage appointments, coordinate support services, allocate resources, and maintain accurate documentation with regular progress updates to ensure participants stay aligned with their goals and budgets.

• Advocate for participant rights and empower participants by educating and supporting them to self- advocate.

• Provide proactive support and rapid coordination during times of crisis or significant change to ensure uninterrupted, high-quality service delivery.

• Facilitate communication and collaboration between participants, Support Workers, and providers for seamless service coordination.

• Monitor service delivery and quality to ensure compliance with NDIS standards and policies.

About You

We are looking for a passionate and experienced Specialist Support Coordinator who is committed to enhancing the quality of life for individuals with disabilities.

Essential Criteria:

• Bachelor's degree in Social Work, Psychology, Community Services, Human Services, Mental Health, or related fields.

• Certificate III in Individual Support (CHC33015), Certificate IV in Disability (CHC43115) or Diploma in Community Services (CHC52015).

• Experience as a Support Coordinator, Case Manager or a similar role with a strong knowledge of the NDIS framework.

• Experience working with participants with complex psychosocial disabilities.

• Demonstrate deep knowledge of complex risk factors.

• Strong communication, empathy, and interpersonal skills.

• Ability to work both independently and collaboratively.

• Understand the role of the mainstream service system.

• Problem-solving and decision-making abilities.

Desirable Criteria:

• Previous experience in a multidisciplinary setting.

• Current driver's license and reliable transport.

• NDIS Worker Screening Check.

• Working with Children Check.

• NDIS Worker Orientation Module Certificate of Completion.

• Valid Police Check.

• Professional Indemnity Insurance.

• Covid-19 Training.

Location & Work Arrangement

• In-Office Appointments – 118 Derrimut Road, Hoppers Crossing, VIC 3029

• Community-Based Support – Assisting participants in their preferred locations

• Remote Support – Providing virtual consultations via Zoom

Job Types: Part-time, Subcontract

Pay: $130.00 – $133.00 per hour
