About The Role
We are seeking multiple experienced APS6
Complex Case Managers (Clinical)
to provide specialised, person-centred support to
veterans and their families
. The role focuses on helping current and former Australian Defence Force personnel navigate complex wellbeing, health, rehabilitation, and social support needs.
You will work with veterans presenting with a range of challenges, including service-related injuries, mental health concerns, trauma, transition issues, and broader psychosocial stressors. Using trauma-informed, evidence-based frameworks, you will coordinate services, advocate for clients, and ensure they receive timely access to entitlements and support pathways.
Key Responsibilities
- Provide end-to-end case management for veterans with complex or high-risk needs, acting as their primary point of contact.
- Conduct intake interviews, psychosocial assessments, risk reviews, and ongoing monitoring of wellbeing.
- Develop tailored case plans focused on capacity building, social participation, rehabilitation, and sustainable independence.
- Support veterans to access clinical, mental health, rehabilitation, housing, financial, or community-based services.
- Manage sensitive and sometimes distressing cases with professionalism and clinical judgment.
- Liaise with health providers, ex-service organisations, community partners, advocates, and internal stakeholders.
- Escalate high-risk situations to senior clinical staff and contribute to coordinated crisis responses when required.
- Prepare high-quality case notes, reports, and stakeholder briefings.
- Provide guidance to colleagues on veteran-specific needs, trauma impacts, and clinical considerations.
- Contribute to continuous improvement of case management practices for the veteran community.
Essential Requirements
- Tertiary qualifications in Psychology, Social Work, Occupational Therapy, Nursing, or a related clinical field.
- Demonstrated experience providing case management or clinical support to individuals with complex, trauma-related, or co-morbid needs.
- Proficiency with case management systems and MS Office.
- Australian citizenship, National Police Check, and ability to obtain a Baseline Security Clearance.
Highly Desirable
- Knowledge of trauma-informed practice, suicide risk considerations, and crisis intervention.
- Familiarity with psychosocial rehabilitation, wellbeing frameworks, or support coordination models.
