We are seeking experienced and clinically trained Complex Case Managers to support a national government program dedicated to improving wellbeing outcomes for individuals with complex support needs.

This is a client-facing role suited for a professional with strong clinical backgrounds, well-developed case management capability and the ability to support clients presenting with mental health challenges, vulnerabilities and multifaceted psychosocial needs.

You will operate within a trauma-informed, person-centred framework, helping clients navigate government services, access entitlements, and connect to appropriate clinical and community supports.

About the Program

The program delivers coordinated, time-limited case management services designed to assist vulnerable clients and their families. The service model incorporates a clinical lens, trauma-informed care and a focus on increasing clients' capacity for independence and wellbeing.

Case management teams operate across Australia and collaborate closely with healthcare providers, community organisations, support networks and internal service delivery areas.

The Role

As an Complex Case Manager -Clinical, you will manage a caseload of clients with multifaceted needs applying your clinical judgment, professional expertise and trauma-informed practice to:

  • Deliver high-quality client-centred case management to individuals with complex presentations.
  • Conduct intake, triage and comprehensive risk assessments, considering clinical, psychosocial and environmental factors.
  • Provide clinical insight and specialised advice to colleagues handling challenging presentations or risk escalations.
  • Serve as a primary point of contact for clients, guiding them through processes and coordinating health, mental health and community services.
  • Manage high-risk matters with appropriate escalation to senior clinicians or leadership when required.
  • Build and maintain strong relationships with internal teams, external providers and community networks.
  • Prepare high-level case summaries, documentation, reports and briefs within the program's frameworks and protocols.
  • Represent the service through stakeholder meetings, presentations and community engagement when required.
  • Contribute to continuous improvement of case management practice and overall service delivery.

This role is suited to professionals who can thrive in a fast-paced environment, operate with autonomy, and maintain compassion and stability when working with clients experiencing significant complexity.

Key Capabilities

The ideal candidate will demonstrate:

  • Strong clinical capability and experience supporting individuals with mental health conditions, disabilities, trauma histories or complex co-morbidities.
  • High-level case management skills, including assessment, planning, coordination, monitoring and review.
  • Understanding of capacity-building frameworks and the impact of health and social factors on wellbeing.
  • Ability to develop positive, productive relationships with clients, stakeholders and multidisciplinary teams.
  • Excellent written and verbal communication with the ability to prepare high-quality client documentation.
  • Strong organisational skills with the ability to manage competing priorities and conduct risk-based decision-making.
  • Proficiency with IT systems including Microsoft Office and case management software.
Mandatory Requirements

To be considered, candidates must have:

  • Tertiary qualifications (Bachelor or higher) in Nursing, Psychology, Occupational Therapy, Social Work or another relevant clinical discipline.
  • Professional experience supporting clients with complex needs.
  • Understanding of disability impacts and capacity-building approaches.
  • Experience in triage, assessment and case management, ideally in a clinical or mental health setting.
  • Strong communication skills, written and verbal.
  • Current Police Check (or willingness to obtain).
  • Australian citizenship.
Highly Desirable Qualifications & Experience
  • Qualifications in mental health, disability, allied health, education or human services.
  • Experience supporting clients with trauma histories or challenging behavioural presentations.
  • Experience working with structured therapeutic or psychosocial interventions.
  • Familiarity with evidence-based practices for individuals with complex support needs.
  • Lived experience or experience working with culturally diverse population groups, including Aboriginal and Torres Strait Islander communities.
