Community Care Officer – Make a Real Difference

About The Role
As a Community Care Officer, you'll play a pivotal role in delivering high-quality Home and Community Care (HACC) services in line with the Aged Care Act 2024. You'll be the bridge between clients, families, and care workers—ensuring services are not only delivered, but continuously improved.
Your Responsibilities Will Include
- Maintaining and managing a computerised rostering system to match care workers with clients.
- Providing guidance, training, and support to Community Care Workers, including home visits when needed.
- Monitoring performance and ensuring compliance with Aged Care Quality Standards, HACC service standards, and OHS requirements.
- Collating and responding to client and carer feedback, driving service improvements.
- Supporting programs such as the Community Meals Program and Senior Citizen Group activities.
- Participating in on-call roster services to ensure clients and carers are supported around the clock.
- Undertaking key administration tasks including training records, newsletters, and resource management.
What We're Looking For
We're seeking someone who brings both heart and skill to the role:
- Minimum Certificate IV in Community Services (or extensive HACC experience).
- Knowledge of person-centred care and the strengthened Aged Care Quality Standards.
- Strong communication, negotiation, and consultative skills.
- Ability to work independently, manage priorities, and exercise initiative.
- Current First Aid/CPR, Driver Licence, and a satisfactory Police Record Check.
