NHW have an exciting opportunity for a passionate and experienced staff member to join our team as a
Sub-acute Ambulatory Care (SACS) Co-ordinator
.

In this vital role, you will provide
day-to-day operational leadership and coordination
across the
SACS and Transition Care Program (TCP)
services. The role is key to delivering high-quality, goal-directed care that supports clients' independence, promotes hospital admission avoidance, and ensures smooth patient flow throughout
Northeast Health Wangaratta (NHW).
As SACS Co-ordinator, You Will

  • Support and guide staff in delivering holistic, person-centred care.
  • Work collaboratively across NHW departments and with external services.
  • Promote NHW as a leading regional health care provider.
  • Lead with compassion, initiative, and a strong team-based approach.

Our Region

  • Wangaratta has a population of approximately 30,000 and our catchment serves a community of around throughout the northeast.
  • An easy 2 ½ hours drive on the Hume freeway from Melbourne and less than an hour to Albury airport with direct flights to most of the large Australian cities.
