**About The Role**:
As a Support Coordinator you will work closely with participants to provide individualised support. You will be responsible for searching, coordinating and implementing the support within an individual’s plan to manage the impact of their disability, to improve their health, well-being and participation within the community.
**Responsibilities**:
The key responsibilities of a Support Coordinator include, but are not limited to:
- Empowering participants by supporting them in making their own choices
- Ensuring participants’ service agreements, service bookings, and all NDIA reporting requirements are managed efficiently and completed on time.
- Monitoring and preparing participant progress reports, including goals, and evaluating the success of provided supports.
- Coordinate required services for participants to support their needs and goals for them to live fulfilling lives within the community.
- Building trusting, positive relationships with participants, their families and service providers while maintaining appropriate and professional boundaries.

- Excellent time management skills
- Ability to prioritize and self-motivate
- Ability to take direction
- Sincere interest in the disability sector and mental health
- Excellent report writing discipline
- Ability to work with culturally diverse communities and individuals
**Required Experience, Skill and Qualifications**:
- Minimum 12 months experience as a Support Coordinator
- Relevant Qualifications including:
- Diploma in Community Services; or
- Cert III in Disability; or
- Cert IV in Disability
- NDIS Worker Screening Clearance
- NDIS Worker Orientation Module Certificate
- A current drivers’ licence
- A roadworthy and registered car with comprehensive car insurance
- Valid Police Check (renewable every three years)
- Working with Children’s Check
Please note it is a condition of employment to be fully vaccination against Covid-19 for this role. Proof of vaccination will be requested prior to commencement.
