The Salvation Army Australia’s Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

Our philosophy is that every person deserves dignity, respect and quality of service and that no one should be without a safe, affordable and secure home. Services are free, voluntary, accredited and delivered by professional and committed staff.

ICMI program offers support to people presenting with high and complex needs to help stabilise their personal circumstances, maintain their housing or assist them out of homelessness.

The role you could play

The ICMI worker also promotes collaborative partnerships between relevant service sectors to reduce the level of complexity associated with multiple service involvement.

You will:

- Conduct comprehensive assessments of the client’s issues, needs and circumstances from initial intake to exit in order to develop collaborative case plans
- Coordinate case conferences with professionals involved with clients to ensure collaborative service delivery
- Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
- Provide case management support to clients
- Create transferable relationships; promote independence and sustainable pathways out of homelessness
- Develop and maintain strong relationships with appropriate services to maximise client access to mainstream and specialist services

This is a permanent part time position, 30.4 hours per week.

What you will bring
- Tertiary qualification in social work or a related discipline is considered desirable
- Current Australian Drivers License is required
- Working With Children Check - Victoria
- A willingness to undergo a National Police Check
