- **Position is open to Nursing or Allied Health staff.**:

- Part time ongoing 64 hours per fortnight.
- Based at our Royal Park Campus, primarily in the Rehabilitation Ward but supporting other wards on site if required.

Looking for a dynamic, self driven and motivated professional to fulfill the part time (.8EFT) role of Progression of Care Navigator (PCN) at Royal Park Campus. Based in the Rehabilitation ward, the PCN role is a key driver in helping to facilitate timely care and patient flow by early identification and prioritisation of discharge planning processes.

**Your New Role**:

- The Progression of Care Navigator (PCN) role is a key driver in helping to facilitate timely care and patient flow by early identification and prioritisation of discharge planning processes.
- The PCN works as a key part of the ward multi-disciplinary team to support the standardisation and improvements in discharge that are reflected in the agreed access metrics for the ward.
- The PCN’s role primarily focuses on routine and complex discharges, identification of safe, appropriate and patient-centred discharge plans, in collaboration with the MDT, patient and their family.

**About you**:

- You are registered as a Division 1 Registered Nurse under the Australian Health Practitioner Regulation National Law Act (2009) with the Nursing and Midwifery Board of Australia OR
- Current registration as an Allied Health Professional with Australian Health Practitioner Regulation Agency.
- At least 5 years clinical experience in sub-acute healthcare and relevant experience and understanding of safe discharge principles.
- Excellent communication skills and ability to engage a wide range of stakeholders.
