**About the role**:

We are currently seeking a Cancer Care Coordinator (CCC) to join our innovative and dynamic team across our Wesley and Chermside Cancer Centres. The Cancer Care Coordinator is responsible for co-ordinating complex care of patients treated in Icon Cancer Centres with the purpose of improving health, treatment and end of life outcomes. The role seeks to expand on our service provision via liaising with other key oncology service providers and assisting patients and families navigate the complex medical and psychological aspects of their treatment.

This is a permanent, part-time opportunity (0.8FTE), operating out of our Wesley centre 2 days per week, and our Chermside centre 2 days per week.

**Key Responsibilities**:

- Assessment and screening for clinical and supportive care needs of people at risk of adverse clinical and psychosocial outcomes
- Ensuring timely and appropriate referral to specialists, allied health providers and support services
- Facilitating continuity of care between health professionals across healthcare settings by establishing clear referral pathways and timely communication
- Providing timely and consistent education and information to patients and their families
- Facilitating delivery of cancer care consistent with established evidence-based guidelines
- Participating in service improvement activities that aim to improve coordination of care and optimise outcomes for individuals and services
- Act as a liaison between patient, Medical Officers and members of the multi-disciplinary health care team
- Development and maintenance of patient information resources and holistic assessment tools
- Development and facilitation of MDTs and improved referral pathways.
- Strengthen service networks and expand service offering.

**Skills & Experience**:

Mandatory:

- Licence to practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
- Bachelor of Nursing
- Post graduate oncology nursing qualification or equivalent
- Minimum 5 years post -graduation nursing experience
- Minimum 3 years clinical experience in oncology nursing
- Expert level clinical knowledge and skills in managing patients with haematological and solid tumour malignancies in the ambulatory and outpatient setting
- Ability to work autonomously and in a team
- Expert level clinical knowledge and skills in managing patients
- Highly effective interpersonal communication and collaborative problem-solving skills

Desired:

- Member of relevant professional bodies (e.g. CNSA, COSA)
