Are you passionate about empowering older Australians to live independently in the comfort of their own homes? We are seeking a compassionate and experienced Personal Care Assistant (PCA) to provide high-quality home-based care under the Commonwealth Home Support Program (CHSP). You'll play a pivotal role in supporting frail, older people to maximise their independence, well-being, and quality of life. This role offers the opportunity to make a meaningful difference in your community while enhancing the lives of those who need it most.

Key Responsibilities:

- Deliver flexible, high-quality care to clients aged 65+ (50+ for Aboriginal and Torres Strait Islander people).
- Work with clients, families, and carers to develop personalised care plans.
- Ensure services are socially and culturally appropriate, meeting individual needs.
- Assist clients to remain in their homes, delaying the need for higher-level care.
- Foster a supportive and empowering environment for clients, ensuring their dignity and choice.

**About You**
- Experience in health/aged care and community settings.
- Proven ability to work autonomously and as part of a multidisciplinary team.
- Strong communication skills and the ability to manage client relationships effectively.
- Competency in time management, conflict resolution, and computer programs (Microsoft Word, Excel, CME).

- National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
- Immunisation Risk for this position is - Category A
