**The Role**
As a Clinical Care Partner, you’ll provide both nursing care and care management for older clients living at home. This is a varied role that combines clinical expertise with building trusted relationships. You’ll split your time between office work and visiting clients in the community (with a company car), ensuring care is person-centred, safe, and empowering.

**What you’ll do**:

- Lead client intake and onboarding, conduct assessments, and develop and review care plans and budgets.
- Provide nursing services such as wound care, medication support, catheter care, and dementia strategies.
- Support and mentor care workers, ensuring seamless delivery of care and continuity for clients.
- Work closely with families, representatives, the care team, internal and external stakeholders, health professionals, and the Aged Care Quality and Safety Commission to achieve the best outcomes for clients.
- Maintain full compliance with the Aged Care Act, program funding guidelines, Quality Standards, and organisational policies.

**About You**

We’re looking for a registered nurse who is compassionate, organised, and thrives on autonomy as well as teamwork.

**Essential**:

- AHPRA registration - Registered or Enrolled Nurse
- Valid Driver Licence (willingness to drive to client homes, company car provided).
- National Police Check (or willingness to obtain)
- Full Australian work rights.

**Desirable**:

- Experience in community or aged care, particularly managing clients with complex needs under Home Care Packages (HCP)
- Strong assessment, planning, and clinical decision-making skills
- Cultural awareness and ability to connect with diverse clients
- Confident with digital care systems and Microsoft Office.
