We are seeking someone who has demonstrated experience as **either a CHSP Coordinator or Home Care Package Coordinator **who genuinely enjoys providing case management services for people over the age of 65 years. You will be responsible for the coordination, care planning and monitoring of CHSP clients to ensure a quality service is provided. You will enjoy building good relationships with clients, their families, other health professionals. You will work closely with our other CHSP and HCP Coordintors and Clinical team. This is a full time position, based in the Brisbane CBD and would suit someone living in the Brisbane North/Caboolture region where the majority of your clients live. A car is available for in-home visits. All Aged Care has been operating for 22 years as a quality approved provider of home care services with Head Office located in the Brisbane CBD close to public transport. We are a multi-cultural organisation who pride ourselves on delivering services which exceed client's expectations which enable clients to remain living independently in their own homes. To be successful in this role you will have: - Minimum of 1 year CHSP or Home Care Packager Coordinator experience AND a Diploma of Community Services qualification or Case Management skill set - Good communication, problem solving and organisational skills - Ability to prioritise and multi-task - Be Self-motivated with a "can do" attitude - Good computer skills including with care databases, Outlook, MS Word and Excel - An ability to speak another language is highly regarded You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. You are required to be fully vaccinated - COVID19 and Influenza. **Salary**: $70,000.00 - $75,000.00 per year **Benefits**: - Salary packaging Schedule: - Monday to Friday **Education**: - Certificate I - IV (required) **Experience**: - CHSP or HCP Coordinator: 1 year (required) Work Authorisation: - Australia (required)
