An exciting opportunity currently exists for an enthusiastic and highly experienced **Registered Psychiatric Nurse - Transitions Coordinator** to join our **Acute Psychiatry Unit.**

The Acute Psychiatry Unit is the acute mental health inpatient unit based at the Austin Hospital, and forms a clincal unit of the Mental Health Division at Austin Health. The Mental Health Division provides care and services through a comprehensive range of services to meet the needs of our clients throughout the North Eastern suburbs of Melbourne.

The **Transition Coordinator** is a senior mental health nurse who works collaboratively with the clinical team of the acute inpatient unit in managing and coordinating the unit bed flow. Reporting to the Divisional Manager, Adult Mental Health Services, the **Transitions Coordinator** plays an important role in access management and discharge planning, and is a member of the Adult Mental Health Service leadership team.
