**About the Role**

The Aged and Community Care is looking for passionate Leader to support the Aged and Community Care Program. The Team Leader will contribute to the overall direction and management of Aged and Community Care Services as a member of the Senior Leadership Team. Ensure the delivery of service performance in line with program objectives and targets in consultation with the Manger. Assist the team in achieving day to day operational service delivery. Provide operational support to staff in conjunction with the manger by developing, implementing, measuring and monitoring set KPI’s

This position is based at Cranbourne and Pakenham but may be required to work at any of the Monash Health sites.
